The inability to become pregnant following one year of unprotected sexual intercourse is most often defined as Infertility. It is recommended that the patient seeks medical evaluation if pregnancy does not occur after one year, and also diagnosis by an infertility specialist.

Treatment options for Women

1) Intra-Uterine Insemination (IUI)

Once known as artificial insemination, in this the sperm is deposited in a woman’s uterus through artificial means. It is also called IUI. This is a less invasive and more affordable alternative to IVF/ICSI for many couples. First the Sperm used in IUI is properly prepared, this process is known as sperm washing. The chances of success with this procedure are influenced by just how good the sperms are prepared. Of course, there are variable factors which can affect how successful this treatment will be for you as with any form of infertility treatment.

2) In-Vitro Fertilization (IVF)

IVF offers the chance of parenthood for the infertile couples who until recently would have had no hope of having a ‘biologically related’ child i.e. where women have blocked or absent fallopian tubes, repeated IUI failure or where men have low sperm counts.

Eggs are surgically removed from the ovary and mixed or injected with sperm outside the body in a Petri dish (‘in-vitro’ is Latin for ‘in glass’) in IVF/ICSI. The eggs are examined after about 40 hours, to see if they have become fertilized by the sperm and are dividing into cells. Then bypassing the fallopian tubes these fertilized eggs (embryos) are placed in the woman’s uterus.

Infertility in Men

Low sperm counts, poor sperm quality, or both attribute to more than 90% of male infertility cases. The cause is unknown in 30% to 40% of cases of sperm abnormalities. One or a combination of factors that include chronic illness, malnutrition, genetic defects, structural abnormalities, and environmental factors may result in infertility in men. . Some of the common factors causing male infertility are:
  • Prostate infections
  • Prior testicle injury
  • Varicocele
  • A history of sexually transmitted disease
  • Problems having intercourse
  • History of infertility
Problems in man’s sperm according to many research available, account for 35-40% of all fertility problems in a couple. Problem in sperm could be classified as:
  • Low sperm or no sperm count
  • Poor sperm motility
  • Abnormally-shaped sperm
Close to 10% of infertile men have immune reactions to sperm as per research studies, and they produce antibodies that kill sperm cells. In men Varicocele (enlarged veins in the testes) is another cause where the veins draining the testes get enlarged & cause decreased oxygenation to the testes thus affecting sperm quality. Due to obstructions in the tubes that transport fertile sperm also Male tube can be blocked.

Tube blockage problem are also associated with sexually transmitted diseases.

The shape of spermatozoa, their motility and their ability to perform the functions necessary for fertilizing an egg define the fertilizing potential of sperm and finally, at the time of fertilization the transfer of an intact genetic material (DNA) to the egg.

The only way to see if fertilization actually occurs if a pregnancy does not occur naturally, is then to take the eggs and sperm out of the body and put them together in the laboratory [in-vitro fertilization - IVF].
